Clinical and Pathological Studies on the Cirrhotic Patients Treated with Portal Hypertension by Shunting Surgery

Abstract
1) This clinical studies consist of ten cases who were admitted to the Presbyterian: Hospital, Taegu from April 1961 to June 1962 because of portal hypertension associated with cirrhosis of the liver. The author has tried to compare the changes in hematology and biochemistry; of the gradient of portal pressure; and in morphology of the liver before and after shunting.
2) The mean postoperative portal pressure is about 263 mm. of water and there seem to be no correlations between the portal pressure and the caliber of the portal vein.
3) It is the author¢¥s opinion that the spleorenal shunt may seem to be safer than the portacaval shunt on those cases who have severe degree of hypersplenism and may develop hepatic coma postoperatively.
4) Most of the cases show pancytopenia and bone marrow hyperplasia probably caused by the longstanding hypersplenism. The follow-up hematological studies reveal good improvement. The changes are more remarkable in the cases treated by splenorenal shunt than in those treated by fortacaval shunt.
4) Splenoportagraphy is of help to evaluate the site of obstruction and the pattern of the portal vein.
5) 6) The follow-up liver function studies appear somewhat improved.
7) The contributing factors as to the causes of cirrhosis of the liver are dubious in most of the cases studied.
8) The histological comparative studies of the liver by biopsy before and after shunting on two cases show no appreciable changes.
